Provide the following items to the Customer Service Department:

• Photo ID and Federal Tax ID Number
• Signed Copy of the Lease if Renting
• Signed Settlement Sheet if Purchasing a property
• Electric Deposit – 2/12 of the average annual estimated bill or a minimum of $500
• Electric Connect Fee - $50
• Trash Deposit - $100 (if the business has City trash service)
• Certificate of Use approval from the Planning Department
• City of Milford Business License

Service can be setup with the City Customer Service Department no more than 7 days in advance
